Description

• Conduct evaluations for patients in inpatient or outpatient environments, crafting individualized treatment plans tailored for recovery
• Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and everyday functioning
• Work collaboratively with nurses, physicians, case managers, and rehabilitation teams to achieve optimal patient outcomes
• Guide patients and their families through therapeutic goals, discharge planning, and home exercise regimens
• Maintain accurate documentation of evaluations, treatments, and progress in compliance with facility and regulatory standards
• Engage in care team rounds and interdisciplinary discussions to coordinate comprehensive patient care

Education

• Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification is preferred
