Key Responsibilities\n

As a Licensed Clinical Social Worker at Cognitive Psychiatric Center, your responsibilities will include:

• Conducting thorough psychosocial assessments of patients and their families to identify emotional, social, and environmental factors impacting their wellbeing.

• Formulating and implementing individualized care plans utilizing appropriate social work interventions to address patients’ needs.

• Collaborating with the broader healthcare team, actively involving patients and families in care plan development and implementation.

• Facilitating discharge planning, coordinating with physicians and other healthcare providers to arrange safe transitions for patients leaving hospice care.

• Providing education and counseling to patients and families regarding adaptation strategies to illness, treatment options, and managing grief.

• Utilizing crisis intervention techniques and delivering brief or long-term individual, group, and family therapies as required.

• Evaluating oncology patients for signs of depression or anxiety and providing short-term counseling services.

• Being on an on-call rotation to handle after-hours inquiries and facilitate support as needed.

• Presenting at educational conferences and workshops about psychosocial oncology practices, establishing yourself as a thought leader in the field.

• Participating in community outreach efforts to strengthen resources available for cancer survivors, offering educational services and consultations.

Your focus will primarily be on hospice patients dealing with terminal illnesses and their families, providing compassionate care to help them navigate complex emotional landscapes. The ideal candidate will possess:

• A Bachelor’s degree in Social Work (or a related field) along with current LCSW licensure in the state of New Jersey.
