Key Responsibilities\n

• Conduct comprehensive assessments to understand the mental health and substance abuse histories of patients seeking treatment at our hospice center.

• Develop tailored treatment plans utilizing evidence-based interventions, such as Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT), to assist patients in their recovery journeys.

• Provide one-on-one psychotherapy sessions, addressing individual needs and ensuring an empathetic approach to promote client engagement and motivation.

• Maintain accurate clinical documentation, adhering to HIPAA regulations and organizational guidelines to ensure compliance and quality care.

• Collaborate effectively with our interdisciplinary treatment team to coordinate care plans and improve patient outcomes.

• Educate patients and their families about resources available for mental health support and recovery, empowering them throughout their journey.

Patient Population / Client Focus\n

You will primarily work with diverse adult patients who are coping with substance abuse issues alongside terminal or chronic illnesses. This role requires a deep understanding of the emotional, social, and psychological impacts of these challenges, enabling you to provide sensitive and informed support to individuals and their families during critical times.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Valid Licensed Clinical Social Worker (LCSW) designation in the state of New Jersey.
