Key Responsibilities\n

• Conduct culturally sensitive and strength-based assessments for new patients and their families, gathering key information to inform treatment plans.

• Provide individualized therapeutic services, including both individual and family therapy, based on the unique preferences and needs identified during assessments.

• Ensure timely completion of essential documentation, including billing, insurance authorization forms, and individualized treatment plans, adhering to compliance standards.
\n
• Develop personalized crisis plans, safety plans, relapse prevention strategies, and wellness plans that support patients in maintaining stable functioning and achieving their treatment goals.

• Create structured discharge plans that facilitate a seamless transition for patients returning to their communities or other care settings.
\n
• Consistently adhere to federal and state regulations, as well as internal policies and procedures, to maintain the highest ethical standards in practice.

• Engage in ongoing professional development through training and continuing education to enhance skills and maintain licensure.
\n
• Participate actively in case consultations and agency meetings to contribute insights and learn from peer experiences.
\n
• Complete all mandatory training requirements to ensure compliance with licensure and organizational standards.

Patient Population / Client Focus\n

In this role, you will be working with a diverse range of patients, including individuals and families facing challenges such as anxiety, depression, trauma, and relationship issues. Qualifications\n

• A master’s degree in social work, counseling, marriage and family therapy, or a related field is required, which meets the criteria for licensure.

• Current licensure as a Mental Health Therapist, specifically as an LMHC, LMFT, or LCSW in the State of New York.

• Experience working in outpatient therapy settings is preferred.

Compensation and Benefits\n
