Work Environment

This position is primarily in the field, where you will travel to patients' homes throughout East Brunswick and surrounding areas. Your day will typically consist of scheduled visits from Monday to Friday, between the hours of 9 AM and 5 PM. The homecare setting allows you to work independently while accessing a wealth of resources, including support from an interdisciplinary team comprising nursing, occupational therapy (OT), speech-language pathology (SLP), and social work professionals. You will leverage various electronic medical record (EMR) systems to document your sessions and track patient progress.

Key Responsibilities

• Conduct thorough in-home physical therapy evaluations to assess patients' capabilities and needs.
• Design and implement individualized treatment plans that address each patient's unique challenges, including rehabilitation from surgical procedures and management of chronic conditions.
• Administer therapeutic exercises, gait training, balance activities, and functional mobility interventions tailored to improve patients' daily living skills.
• Educate patients and caregivers on home safety, appropriate exercise techniques, fall prevention methods, and wellness strategies to empower them in their recovery process.
• Continuously monitor patient progress and adjust treatment plans to meet evolving functional goals; maintain clear communication with patients and their families.
• Document all patient interactions and treatment notes accurately in accordance with Medicare and agency documentation guidelines.
• Collaborate with a multi-disciplinary team to share information and ensure coordinated patient care.

Patient Population / Client Focus

As a Home Health Physical Therapist, you will work with a diverse patient population, primarily comprising individuals recovering from surgery, managing orthopedic issues, neurological conditions, and the elderly requiring rehabilitation services. You will interface closely with patients in their home environments, facilitating a unique understanding of their daily challenges and strengths. This hands-on approach fosters not only recovery but empowers patients to enhance their quality of life.

Qualifications

• Current and valid New Jersey Physical Therapist License is required.
• Bachelor's Degree or higher in Physical Therapy from an accredited program (DPT or MPT preferred).
• A valid driver's license, dependable vehicle, and car insurance are mandatory, as this position requires travel to patient homes.
• CPR/BLS certification.
• Strong clinical judgment, exceptional communication skills, and the ability to manage time effectively in a dynamic work setting.
• Must be comfortable working independently and making decisions regarding patient care without immediate supervision.

Preferred Experience

• A minimum of two years of experience in home health, outpatient, or hospital-based rehabilitation settings is preferred.
• Familiarity with EMR systems, such as HomeCare HomeBase, Kinnser, or MatrixCare would be beneficial.
• Understanding of Medicare documentation requirements, including OASIS standards, is a plus; training provided for those who are willing to learn.
