Key Responsibilities\n

• Conduct thorough assessments of patients' mental health to develop appropriate treatment strategies that address their individual needs.

• Collaborate with supervisors, family members, guardians, and other stakeholders to communicate patient progress and treatment outcomes, ensuring holistic support.

• Create and maintain a tentative schedule for client appointments, home visits, and observation periods to track therapeutic advancements.

• Coordinate and perform home visits for observational assessments, chart reviews, and interviews to provide comprehensive support to clients in their home environments.

• Participate in interdisciplinary team meetings to discuss treatment plans, share insights, and refine care strategies.

• Document monthly and quarterly progress notes to track client advancement and treatment effectiveness.
\n
• Organize and lead training sessions to enhance the competencies of the clinical staff in implementing treatment plans and maintaining documentation standards.

• Act as a liaison with community partners to foster collaboration in mental health services and facilitate resource allocation.

• Produce training materials and standard operating procedures to streamline practice efficiencies and enhance program offerings.
