Key Responsibilities\n

• Work directly with clients experiencing major mental illnesses or severe emotional disorders, offering individual, group, and family therapy.

• Develop and implement integrated care plans tailored to each client’s needs, ensuring continuity of care and coordination with other healthcare providers.
\n
• Participate actively in team meetings, contributing to case discussions and sharing insights to enhance team performance and client care.
\n
• Conduct assessments and screenings, utilizing evidence-based practices to determine the best course of action for clients.
\n
• Engage in crisis intervention efforts, providing immediate support and intervention when necessary.
\n
• Monitor caseloads, ensuring that clients receive timely and appropriate treatment services focused on their immediate concerns and aspirations.

• Maintain ethical standards and guidelines set forth by professional associations and Young Minds Counseling P.C.
\n
• Complete necessary documentation and reports promptly, ensuring data accuracy for quality improvement initiatives.

• Contribute to interdisciplinary collaboration by sharing important information with family members, caregivers, and other stakeholders.

• Assist with on-call duties as assigned, providing necessary support during and beyond regular program hours.

Patient Population / Client Focus\n

The primary client population served includes individuals who are in hospice care, dealing with life-limiting illnesses and the psychological challenges that come with them. Your role will involve providing emotional support, counseling, and necessary interventions that cater to the unique needs of these clients and their families.

Qualifications\n

• Must have a Bachelor’s Degree in Social Work; a Master’s Degree is preferred.

• Current and valid LCSW license in the state of Kansas.

• A minimum of two years of relevant clinical experience.

• Strong understanding of mental health disorders and their impacts on clients' lives.

• Proficiency in creating personalized care plans that address mental and emotional health needs.

Compensation and Benefits\n

This full-time position offers a competitive salary range of USD 70,000 to 85,000 annually, commensurate with experience.
