This full-time role is based in West Palm Beach, Florida, where you will work onsite, providing hands-on treatment and rehabilitation services. Work Environment

The Physical Therapist position is primarily based in the home care setting, providing an intimate and engaging environment that allows for personalized patient interaction. You will work Monday through Friday, from 8 AM to 5 PM. As part of our multidisciplinary team, you'll collaborate with other healthcare professionals to ensure patients receive comprehensive care tailored to their unique needs. Key Responsibilities

• Conduct thorough evaluations of patients, which includes reviewing medical histories and performing specific assessments to develop individualized treatment plans.
• Implement and demonstrate advanced treatment protocols based on patient evaluations while adjusting care plans in response to patient progress and changes in health status.
• Document patient care activities meticulously, including initial evaluation results, ongoing treatment notes, interim progress summaries, and discharge evaluations.
• Communicate effectively with patients, their families, and healthcare team members, ensuring all parties are informed and involved in the rehabilitation process.
• Facilitate discharge planning and follow-up care, providing clear written instructions and recommendations for ongoing rehabilitation needs.
• Attend and engage in team meetings and professional development opportunities to promote continuous improvement within our facility.

Patient Population / Client Focus

In this role, you will primarily work with a diverse patient population, including individuals recovering from orthopedic surgery, sports injuries, and chronic pain conditions. Your expertise will guide patients through rehabilitation, empowering them to regain mobility, strength, and independence. You will play an integral role in developing not just physical wellness, but also enhancing the overall quality of life for each patient.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) is required.
• Current state licensure as a Physical Therapist is necessary.
• Certification in Basic Life Support (BLS) from the American Heart Association is mandatory.

Preferred Experience

• A minimum of 2 years of experience as a practicing Physical Therapist, preferably in outpatient or home care settings.
• Experience collaborating with multidisciplinary teams and applying evidence-based guidelines in treatment protocols.

Compensation and Benefits

Headstrong Rehabilitation & Wellness offers a competitive salary range of USD 100,000 to 120,000 annually, commensurate with experience, along with a robust benefits package that includes:
