Key Responsibilities\n

As a Licensed Professional Counselor, your role will encompass a variety of essential tasks:

• Assist primary care providers in identifying, treating, and managing mental health and psychosocial issues, contributing significantly to the primary care team.

• Conduct comprehensive patient intakes to perform diagnostic evaluations, developing treatment recommendations tailored to each patient’s unique goals.

• Deliver brief, focused interventions for patients requiring mental health services, collaborating with primary care providers to optimize patient treatment plans.

• Perform follow-ups to assess patient progress, modifying treatment approaches as necessary based on ongoing evaluations.

• Develop relapse prevention plans where applicable, aiding patients in stabilizing their conditions and maintaining effective functioning.

• Identify “at risk” patients, working proactively to design preventative treatment strategies to avert worsening mental health conditions.

• Monitor and coordinate behavioral health care delivery, linking with external treatment providers upon patient approval.

• Thoroughly document patient progress and diagnostics in treatment charts, ensuring comprehensive communication with the healthcare team.

• Educate patients about self-management skills, empowering them to take an active role in their recovery and treatment processes.

• Counsel on strategies assisting patients in adhering to prescribed medical treatments alongside emotional and mental health support.

Patient Population / Client Focus\n

This role primarily focuses on adult patients with diverse mental health needs, including those experiencing anxiety, depression, trauma, and other behavioral health challenges. Counselors will address the complexities of mental health issues while considering the social determinants affecting each patient. You will play a vital role in providing culturally sensitive care and support, ensuring that patients feel heard, understood, and empowered to achieve their wellness goals.

Qualifications\n

The ideal candidate will meet the following requirements:

• A Master’s Degree in Clinical Counseling or a related field.

• Current LPC (Licensed Professional Counselor) License in Tennessee.

Preferred Experience\n

Compensation and Benefits\n

The compensation package for this position ranges from USD 87,000 to 105,000 annually, commensurate with experience and qualifications.
