Key Responsibilities\n

• Conduct in-depth psychosocial assessments of seniors to evaluate their emotional, mental, and social needs, using standardized assessment tools.

• Develop and implement individualized care plans, collaborating closely with clients and their families to ensure they reflect the client's preferences and goals.

• Provide ongoing counseling and emotional support to seniors and their families, facilitating sessions to help them cope with life transitions, such as relocation to care facilities or end-of-life decisions.

• Advocate for clients’ rights, actively working to connect them with community resources such as support groups, financial aid services, and counseling programs.

• Regularly conduct home visits to monitor clients' emotional and physical well-being, addressing any safety concerns or changes in their living environment.

• Facilitate family meetings to promote communication and problem-solving among family members involved in the client's care process.

• Maintain accurate and comprehensive documentation of client interactions, care plans, and progress notes to comply with legal and ethical standards.

• Stay informed about current advancements in social work practices and updated regulations that impact elder care, ensuring ethical compliance in all interactions.

Patient Population / Client Focus\n

Your primary focus will be on seniors receiving care at our Hospice Center, who may be experiencing various emotional and psychological challenges associated with aging, serious illness, or end-of-life issues. This role requires sensitivity and a deep understanding of the aging process, compassion and respect for the dignity of every client, and a commitment to improving their quality of life during difficult times.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW).

• A minimum of two years of relevant social work experience, particularly in geriatric care or community-based settings.

• Strong understanding of older adult issues, social work principles, and community resources available to elderly populations.

• Exceptional interpersonal, communication, and counseling skills to establish rapport with clients and provide empathetic support.

Compensation and Benefits\n
