Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate clients' mental health concerns, strengths, and available resources.

• Provide personalized individual, family, and group therapy sessions utilizing evidence-based modalities, such as cognitive-behavioral therapy, trauma-informed care, and solution-focused strategies.

• Develop tailored treatment plans, identifying specific goals and therapeutic interventions designed to meet clients' unique needs.

• Facilitate group therapy sessions and psychoeducation classes to support clients and their families through grief, loss, and other emotional challenges related to hospice care.

• Engage in regular case collaboration with interdisciplinary teams, including physicians, nurses, and other healthcare providers, to deliver holistically coordinated care.

• Provide crisis intervention services and risk assessments, developing safety plans when necessary to safeguard clients’ well-being.

• Maintain detailed, accurate, and timely documentation of client interactions and treatment progress, adhering to ethical and legal standards within social work practice.

• Participate in ongoing professional development and training opportunities to stay abreast of industry best practices and enhance therapeutic skills.

Your role as an LCSW will primarily focus on supporting clients receiving hospice care, as well as their families. Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited institution.
