As a remote position, this role offers flexibility while maintaining regular working hours from Monday to Friday, 9:00 AM to 5:00 PM. Key Responsibilities\n

• Conduct comprehensive bio-psychosocial assessments that consider age-related and cultural factors to address mental health, substance abuse, and medical needs within a primary care setting.

• Provide individualized behavioral health services, including counseling for individuals, couples, and families. This includes conducting intakes, assessments, developing treatment plans, diagnosing, implementing crisis interventions, and coordinating referrals for behavioral health and substance use treatment.

• Document mental health evaluations, progress notes, and treatment plans electronically, ensuring compliance with organizational standards and best practices.
\n
• Identify patient needs and escalate care provisions to medical providers as necessary, ensuring a holistic approach to patient care.
\n
• Serve as a liaison to local resources and community agencies, supporting clients in accessing medication assistance programs and other essential services.
\n
• Collaborate with Quality Improvement teams to enhance service delivery by developing and executing processes for efficient data collection and management.

Patient Population / Client Focus\n

In this role, you will work with a diverse clientele, including adults, children, and families facing various challenges related to mental health and substance use. The ability to conduct assessments and provide treatment within a culturally competent framework is essential. This ensures equitable access to care and tailored interventions that resonate with each patient's unique background and experiences.

Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Current and valid state license as a Licensed Clinical Social Worker (LCSW) in California.

• A minimum of three (3) years of post-licensure experience in a clinical setting.

• Solid understanding of counseling techniques, crisis intervention, and the ability to make independent judgments regarding the delivery of direct services.

Compensation and Benefits\n

The compensation for this position ranges from USD 115,000 to 130,000 annually, commensurate with experience.
