Key Responsibilities\n

• Conduct thorough assessments and regular evaluations of patients to address their unique mental health needs and treatment goals.

• Facilitate effective communication among various stakeholders, including supervisors, direct care staff, family members, and guardians, to keep all parties informed about patient progress.

• Create and share a tentative treatment plan with supervisors, outlining appointments, home visits, and observation sessions to track patient progress.

• Coordinate home visits as necessary, which may include in-home observations, chart reviews, staff interviews, and comprehensive data analysis.

• Participate actively in Interdisciplinary Team Meetings to contribute insights about patient care and treatment strategies.
\n
• Document monthly and quarterly progress reports to monitor developments and assess the effectiveness of interventions.
\n
• Organize and conduct training sessions for staff to enhance their proficiency in executing and documenting program plans effectively.
\n
• Perform follow-up interviews, home observations, and other tracking activities to ensure ongoing adherence to treatment plans.

• Maintain accurate records and manage patient information to ensure accessibility for team members and compliance with legal standards.

• Act as a liaison between the general community and the mental health sector, promoting awareness and understanding of mental health resources.

• Contribute to the development of training materials aimed at improving patient care and staff effectiveness.

Qualifications\n

• A Master's or Doctorate in Counseling, Psychology, Social Work, or a related field

Compensation and Benefits\n
