Key Responsibilities\n

• Conduct ongoing assessments to evaluate patient needs, ensuring that care is tailored to meet individual circumstances.

• Communicate effectively with supervisors, direct care staff, family members, and other stakeholders regarding client progress and treatment plans.

• Create and manage a tentative schedule for appointments, home visits, and follow-ups, closely monitoring workflow and patient engagement.

• Coordinate home visits to meet compliance with program requirements and contribute to comprehensive evaluations through chart reviews and data analysis.

• Guarantee that the implementation of treatment plans is both effective and accurately executed, thereby enhancing patient outcomes.

• Participate in Interdisciplinary Team Meetings to foster collaboration across specialties and contribute to holistic client care.

• Document monthly and quarterly progress notes, ensuring that records are current and accessible to facilitate seamless care transitions.

• Organize and lead training sessions for staff to improve competencies in program implementation and documentation techniques.
\n
• Conduct follow-up interviews and home visits for ongoing support to address evolving client needs.

• Serve as a liaison with the public and mental health community, promoting awareness and support of developmental goals.

• Assist in developing procedures and training materials aimed at continually enhancing programs for clients.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is mandatory.
