Key Responsibilities\n

• Perform comprehensive assessments to identify the needs of your clients, revising treatment plans as necessary to ensure optimal outcomes.

• Schedule and conduct home visits or office appointments to provide personalized care and observe clients in various environments.

• Ensure the effective implementation of the treatment program plans, making necessary adjustments based on ongoing evaluations.

• Participate in Interdisciplinary Team Meetings, sharing insights and contributing to collaborative care discussions.

• Document monthly and quarterly progress notes meticulously to support clinical and administrative decision-making.

• Lead training initiatives to enhance the competency of fellow staff in program plan execution and record-keeping standards.

• Conduct follow-up interviews and observations to support and monitor referrals and client progress effectively.

Patient Population / Client Focus\n

This role involves direct interaction with diverse populations, including both adults and youth. Qualifications\n
