Key Responsibilities\n

• Conduct ongoing assessments to identify and address the diverse needs of your clients, employing evidence-based practices.

• Participate in regular communication with supervisors, direct care staff, and supportive family members to collaboratively discuss client progress and treatment adjustments.

• Prepare and provide a structured schedule of your counseling appointments, including virtual sessions, home visits, and necessary follow-up observations.

• Coordinate home visits and interactions to conduct thorough observations, chart reviews, and client interviews, ensuring comprehensive care.
\n
• Implement and monitor the effectiveness of individualized treatment plans, making adjustments as required to meet evolving client needs.
\n
• Engage in interdisciplinary team meetings to discuss case progress and share insights to enhance patient care.
\n
• Document detailed monthly and quarterly progress notes reflecting client advancements and challenges.
\n
• Organize and facilitate training sessions to ensure that staff and team members are well-equipped to implement care plans and maintain accurate documentation.

• Provide ongoing support through follow-up interviews and home visits, reinforcing client care strategies.

• Maintain meticulous records that ensure information accessibility for all stakeholders involved in client care.

Patient Population / Client Focus\n

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• Active and unrestricted licensure as a Clinical Professional Counselor (LCPC) in the state of Kansas.

• A minimum of two years of mental health counseling experience, preferably in a clinical environment.

• Specialization in areas such as family therapy, couples therapy, substance use, or trauma is highly desirable.

• In-depth understanding of legal and ethical standards within the mental health field.
