Key Responsibilities\n

As a Licensed Clinical Social Worker, you will be responsible for:

• Providing tailored counseling and crisis intervention to patients and their families, addressing challenges such as grief, adjustment disorders, and emotional well-being.

• Collaborating closely with an interdisciplinary rehabilitation team, including physical and occupational therapists, to create comprehensive, individualized treatment plans that incorporate psychosocial elements.

• Facilitating access to community resources, support groups, and vocational training to assist patients in their recovery journeys.

• Educating patients and families about available community resources and post-rehabilitation options to ensure a smooth transition upon discharge.

• Involved in discharge planning, helping patients navigate the transition to home or other appropriate care settings.

Compensation and Benefits\n
