Care Navigator for Complex Kidney Care (Hybrid)

DaVita Integrated Kidney Care (IKC) is seeking a Care Coordinator in Los Angeles for a per diem, 23-hour week schedule. You will support patients with chronic conditions, coordinate referrals, and work with nurses to move care forward from home when possible. The role requires strong communication, confidentiality, and proficiency in MS Office.
