Key Responsibilities\n

• Act as a vital liaison between clients, families, and outpatient providers, ensuring a cohesive approach to care.

• Communicate with insurance companies regarding pre-certification and manage utilization processes to facilitate access to necessary services.

• Engage actively in discharge planning and aftercare coordination, aligning client needs with available resources.
\n
• Provide therapeutic meal support, modeling healthy relationships with food while adapting interventions based on individual client needs.
\n
• Deliver empathetic and affirming care that respects and acknowledges the unique backgrounds and identities of each client.

• Utilize clinical skills to assess client needs and document progress in accordance with established care plans.

• Participate in team meetings to discuss client status, share insights, and develop strategies that enhance overall service delivery.

Patient Population / Client Focus\n

The primary focus of this role is on older adults receiving hospice care. Your work will center around providing empathetic support to clients and their families during a profoundly sensitive time in their lives. Understanding and addressing the emotional complexities of end-of-life issues is crucial, and you will play a key role in helping clients navigate these challenges through individual counseling and group sessions.

Qualifications\n

• Master's degree in Social Work (MSW) from an accredited institution.

• A valid Licensed Clinical Social Worker (LCSW) license in the state of California.

Compensation and Benefits\n
