Key Responsibilities\n

• Conduct thorough assessments to determine the mental health needs of clients, ensuring individualized treatment plans are developed and implemented.

• Maintain regular communication with supervisors and interdisciplinary team members to discuss client progress, treatment modifications, and follow-up strategies.
\n
• Provide clear and organized scheduling of counseling sessions and any necessary home visits to monitor client progress effectively.

• Coordinate home visits when required, conducting in-home observations, chart reviews, and staff interviews to gather crucial data.

• Ensure the program plans are executed efficiently and monitor their effectiveness to facilitate impactful outcomes for clients.

• Participate actively in interdisciplinary team meetings, contributing insights and updates on client statuses and treatment approaches.

• Document and complete monthly and quarterly progress notes that reflect each client's journey and treatment efficacy.
\n
• Organize training and certification for team members on effective strategies and documentation protocols regarding program plans.
\n
• Provide follow-up care through interviews, home visits, and observations, ensuring continuity of support for client needs.

• Keep meticulous records and ensure information is accessible to all relevant stakeholders while safeguarding client confidentiality.

• Act as a liaison with the broader mental health community and the public, assisting in resource development and advocacy initiatives.

• Contribute to the refinement of training materials and procedures aimed at enhancing the efficacy of our treatment programs for individuals.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• Active and unrestricted licensure as a Licensed Professional Counselor (LPC) in the state of Oregon.

• At least two years of counseling experience in a clinical context, demonstrating comprehensive case management skills.

• Strong knowledge of legal and ethical standards governing mental health practices.
