Spring Leaf Rehabilitation Center is seeking a dedicated and skilled Clinical Physical Therapist specializing in orthopedic care to join our team. Work Environment

This position is fully onsite, which means you will be visiting clients in their homes throughout Freehold, New Jersey. You will collaborate closely with interdisciplinary teams and healthcare professionals, allowing for a rich, integrated approach to client care. Key Responsibilities

• Direct physical therapy treatment toward disability prevention, pain relief, muscle function enhancement, and performance maximization in alignment with each client's care plan.
• Supervise and guide physical therapy assistants, ensuring the appropriate implementation of treatment plans and adherence to best practices.
• Conduct thorough evaluations of clients' functional abilities through assessments of muscles, nerves, joints, and overall physical capability.
• Assess the home environment for its suitability as a rehabilitation space, recommending modifications to enhance client independence.
• Coordinate and document physical therapy services as prescribed by physicians, ensuring compliance with clinical standards.
• Administer therapeutic exercises, providing both manual and non-manual instruction to support client compliance in achieving therapy goals.
• Evaluate, fit, and adjust prosthetic and orthotic devices, collaborating with physicians on necessary modifications.
• Engage in ongoing development and revision of care plans in review meetings with interdisciplinary team members.
• Monitor and document shifts in client conditions and responses to therapy, actively making recommendations for care adjustments.
• Maintain comprehensive clinical records, including initial evaluations, progress notes, and discharge summaries, submitted in a timely manner.
• Consult with clients and their families, ensuring they understand the therapy process and empowering them in their own care.
• Facilitate communication among team members regarding client progress and collaborative treatment strategies.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program is required; a Doctorate in Physical Therapy is preferred.
• Current PT licensure in good standing in New Jersey.
• CPR certification and a valid driver's license with automobile insurance are mandatory.
• Exceptional interpersonal communication skills with a strong ability to engage clients and their families.
• A proactive approach to clinical excellence, quality, and integrity in all aspects of care.
• Ability to lead and inspire team members through a positive attitude and collaborative spirit.
• Proficiency in understanding and navigating federal and state healthcare regulations.

Preferred Experience

• Minimum of one year of experience as a physical therapist, preferably in acute care or home care settings.
• Experience providing patient education and demonstrating physical therapy techniques effectively.
• Demonstrated capacity for critical thinking and decision-making in a clinical environment.

Compensation and Benefits
