Work Environment

The Nurse Practitioner will work onsite at our Assisted Living Facility, collaborating closely with a multidisciplinary team of healthcare providers to address the needs of our residents and patients. The clinic offers a compassionate and patient-centered atmosphere where individuals can thrive. Regular operational hours are Monday to Friday, with flexible start times of either 8 am to 4 pm or 9 am to 5 pm, ensuring work-life balance for our staff.

Key Responsibilities

• Diagnose, manage, and treat acute, episodic, and chronic health conditions, ensuring individualized care for each patient.
• Conduct thorough physical examinations, evaluate patient histories, and perform diagnostic assessments to formulate care plans.
• Prescribe medications and implement treatment modalities based on evidence-based guidelines and individual patient needs.
• Develop and refine patient management policies and procedures to enhance healthcare delivery and patient outcomes.
• Facilitate referrals to appropriate specialists and healthcare facilities when necessary to ensure comprehensive care.
• Communicate effectively with collaborative physicians and specialists to coordinate care and address patient concerns.
• Stay current with advancements in medical practices and regulations to provide the highest standard of care.

Patient Population / Client Focus

This role primarily serves residents of the Assisted Living Facility, who may present with various medical conditions ranging from common illnesses to more chronic health issues. As a Nurse Practitioner, you will play a crucial role in managing the healthcare of elderly patients, addressing their unique needs and promoting healthy aging.

Qualifications

• Master's degree in Nursing with specialization as a Nurse Practitioner.
• Current and active Nurse Practitioner license in the state of New York.
• A minimum of 2 years of clinical experience as a Nurse Practitioner in a health care setting.
• Nationally recognized certification for Nurse Practitioners with prescriptive authority.
• Strong knowledge base in medical terminology, pharmacology, and best practice standards.
• Exceptional interpersonal and communication skills, coupled with a knack for teamwork and collaboration.
• A professional demeanor with the ability to connect with patients and their families compassionately.

Preferred Experience

• Experience in urgent care or geriatric health is highly preferred.
• Demonstrated ability to manage complex patient cases and coordinate care across different healthcare settings.
• Familiarity with electronic health record (EHR) systems and other clinical documentation tools.
