Key Responsibilities\n

• Collaborate closely with a diverse care team to develop, implement, and modify personalized treatment plans based on each patient's specific needs and preferences.

• Engage in collaborative case discussions with interdisciplinary medical and social work teams to enhance therapeutic strategies and interventions, leading to improved patient outcomes.

• Provide emotional support, counseling, and education to clients and their families, assisting them in navigating difficult decisions and connecting them with vital community resources and services.

Patient Population / Client Focus\n

In this role, you will primarily serve older adults facing terminal illnesses and their families. Qualifications\n

• A Master’s degree in Social Work or a related field is required for this position.

Compensation and Benefits\n
