Key Responsibilities\n

• Provide counseling services, including crisis intervention for patients experiencing emotional and behavioral issues, ensuring timely assessment and treatment.

• Maintain clear communication with physicians and interdisciplinary team members about patient progress, addressing any concerns or changes in treatment needs.

• Document comprehensive clinical and progress notes in accordance with department policies to facilitate accurate patient records keeping.

• Collaborate closely with healthcare professionals to develop and adjust treatment plans based on psychosocial assessments and standardized interventions.

• Forge productive relationships with community resource leaders, representing Empower Well Health Care Center in various meetings and committees to address the social and psychological needs of our patients.

Patient Population / Client Focus\n

Your role will involve working with a diverse patient population, including individuals experiencing mental health disorders, substance use issues, and a variety of psychosocial stressors. You will provide empathetic, culturally competent services, recognizing the unique challenges faced by each patient in their recovery journey.

Qualifications\n

• A Master’s degree in Social Work, Psychology, Marriage and Family Therapy, or Mental Health Counseling.

• Possession of one of the following independent licenses: Licensed Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).

• A minimum of two years of relevant professional experience in a clinical setting.

Preferred Experience\n

While not mandatory, experience in community health settings or outpatient mental health services is highly regarded. Familiarity with electronic health record (EHR) systems is also advantageous. Compensation and Benefits\n
