Key Responsibilities\n

• Conduct comprehensive psychosocial evaluations and assessments to determine the needs and concerns of our senior clients and their families.

• Develop individualized care plans in conjunction with interdisciplinary teams, facilitating ongoing communication and collaboration with healthcare providers to monitor patient outcomes.

• Offer emotional support and counseling to clients and families, helping them navigate the complexities of end-of-life decisions and bereavement.

• Provide education and resources to families about available services and community resources, ensuring they have access to the support they need.

• Document all interactions and clinical assessments accurately in compliance with state and federal regulations.

This role primarily focuses on seniors and their families who require hospice care. Qualifications\n

• A Master's Degree in Social Work (MSW) or a related field is required.

Compensation and Benefits\n
