Key Responsibilities\n

• Develop, coordinate, and implement individualized support plans based on detailed assessments of each client's unique situation.

• Conduct thorough interviews with clients to evaluate their conditions, identify needs, and recognize their strengths and vulnerabilities.

• Set and monitor measurable goals tailored to each case, providing ongoing adjustments to treatment plans as necessary.

• Utilize evidence-based practices to guide clients through therapy sessions, addressing their concerns, emotional health, and overall well-being.

• Maintain accurate and up-to-date records of client interactions and progress to ensure effective treatment outcomes.

• Participate in case conferences and offer professional insights and testimony when required.

• Serve as the primary point of contact (key-worker) for clients, collaborating effectively with multidisciplinary teams to enhance client care.

• Refer clients to appropriate community services and resources to aid in their recovery journey.

Qualifications\n

Compensation and Benefits\n
