Key Responsibilities

• Conduct comprehensive evaluations of patients in a home care environment to assess their physical abilities and rehabilitation needs.
• Develop, implement, and modify individualized treatment plans based on patient evaluations, using evidence-based therapy methods such as therapeutic exercises, manual therapy, and advanced modalities.
• Track and document treatment outcomes, patient responses, and progress in compliance with professional standards and regulations.
• Reassess and adjust therapeutic goals at regular intervals to enhance patient recovery trajectories effectively.
• Guide patients and their families in the use of assistive devices and self-care strategies to facilitate their mobility and independence.
• Provide comprehensive training and resources for patients, caregivers, and collaborating healthcare personnel on physical therapy techniques and safety protocols.
• Stay current with professional developments and enhance personal expertise through workshops, continued education courses, and networking.

Patient Population / Client Focus

This position directly serves a diverse range of patients recovering from various conditions, including post-surgical rehabilitation, sports injuries, neurological disorders, and chronic pain syndromes. You will work closely with patients of all ages, often collaborating with families to ensure a supportive and informed approach to rehabilitation, which is critical for effective outcomes.

Qualifications

• A Doctorate in Physical Therapy (DPT) is required.
• Current and valid state licensure for Physical Therapy in Florida.
• Excellent verbal and written communication skills.
• Proficient in using electronic health record (EHR) systems and basic computer applications.

Preferred Experience

Compensation and Benefits
