Position Overview

PhysioSpace LLC is actively seeking a dedicated and skilled Physical Therapist to join our dynamic team within a Skilled Nursing Facility (SNF) located in the vibrant community of Centreville, Virginia. This full-time position offers the opportunity to provide top-tier rehabilitation services to residents who are recovering from injuries, managing chronic illnesses, or seeking to improve their mobility and independence. You will work Monday through Friday from 9 AM to 5 PM, ensuring a structured workflow while delivering impactful therapy to those who need it most.

Key Responsibilities

As a Physical Therapist at our Skilled Nursing Facility, your day-to-day responsibilities will include:

• Conducting thorough initial evaluations to assess each resident's mobility, strength, and functional capabilities, which will guide their personalized treatment plans.
• Creating and implementing tailored treatment strategies focusing on pain management, enhancing mobility, and preventing falls through evidence-based practices.
• Administering various therapeutic interventions, such as manual therapy, therapeutic exercises, and balance training to promote recovery and improve functional outcomes.
• Utilizing and recommending assistive devices and mobility aids to empower residents, enabling them to achieve greater independence in their daily activities.
• Collaborating with interdisciplinary teams, including nursing and occupational therapy staff, to ensure coordinated and holistic care for each resident.
• Providing education to residents and their families on home exercises, mobility techniques, and safety precautions to encourage ongoing recovery beyond the facility.
• Maintaining accurate and timely documentation of patient progress, in compliance with SNF regulations and standards, to support quality and accountability in care.

Patient Population / Client Focus

Your work will primarily focus on our senior resident population, many of whom require assistance to regain mobility after surgeries, strokes, or extended hospital stays. This demographic often presents unique challenges such as comorbidities and varying levels of cognitive function. Therefore, you will need to utilize your clinical expertise to adapt therapies that are both effective and considerate of their individual physical and emotional needs.

Qualifications

To qualify for this role, candidates must possess the following:

• An active and valid Physical Therapy license in the state of Virginia.
• A Doctor of Physical Therapy (DPT) degree; alternatively, a Bachelor's or Master's degree in Physical Therapy from an accredited institution.
• Evidence of strong manual therapy and therapeutic exercise skills that demonstrate competence in delivering high-quality care.
• Proven ability to maintain collaborative relationships with team members and communicate effectively with residents and their families.
• A compassionate, patient-centered approach that prioritizes the needs and dignity of our residents.

Preferred Experience

While the minimum requirement is two years of experience, preference will be given to candidates with:

• Previous experience working in a Skilled Nursing Facility or within geriatric rehabilitation settings.

Compensation and Benefits

We offer a competitive salary ranging from $90,000 to $120,000 per year, commensurate with experience, along with a comprehensive benefits package that includes:
