Kinetic Revive Physical Therapy is seeking a dedicated Full-Time Physical Therapist (PT) to join our team at our Skilled Nursing Facility in Centreville, Virginia. Work Environment

This on-site role requires you to actively engage with residents in our rehabilitation facility, providing a dynamic and hands-on work environment. You will collaborate closely with a multidisciplinary team, including nurses and occupational therapists, to ensure holistic and integrated patient care. Your working hours will be Monday to Friday, from 9 AM to 5 PM, enabling you to have a structured schedule that promotes work-life balance.

Key Responsibilities

• Conduct thorough evaluations of residents' physical capabilities to establish clear and achievable therapy goals.
• Design and implement individualized treatment plans aimed at restoring mobility, reducing pain, and enhancing functional autonomy.
• Deliver a variety of treatment modalities, including therapeutic exercises, balance training, and mobility interventions.
• Educate residents and their families about effective mobility practices and provide home exercise programs to support continued progress.
• Collaborate with other healthcare professionals in the facility to create cohesive care plans that foster patient success.
• Document treatment plans, resident progress, and outcomes meticulously, adhering to facility regulations and standards.

Patient Population / Client Focus

You will primarily work with elderly residents who require skilled nursing care, many facing challenges related to mobility, pain management, and rehabilitation from surgeries or injuries. Understanding the unique physical and emotional needs of geriatric patients will be crucial in delivering effective therapy that meets their specific conditions and enhances their quality of life.

Qualifications

• Bachelor's or higher degree in Physical Therapy from an accredited institution.
• A valid Virginia PT license in good standing is mandatory.
• Proficient knowledge of geriatric physiology and rehabilitation practices is preferred.
• Ability to communicate effectively with patients, families, and the healthcare team is essential.

Preferred Experience

While a minimum of 2 years of experience is required, candidates with prior experience in skilled nursing or geriatric care settings will be favored.
