Key Responsibilities

• Conduct thorough psychosocial assessments to evaluate patients' emotional, social, and environmental strengths and challenges, ensuring tailored intervention strategies.
• Collaborate with a multidisciplinary health care team to develop and implement individualized care plans, incorporating the insights and participation of patients and their families.
• Facilitate effective discharge planning, coordinating with physicians, insurers, and other providers to ensure smooth transitions from hospital to home or other care settings.
• Offer education and counseling services to patients and families, helping them adapt to illnesses or life changes and addressing their associated emotional needs.
• Utilize crisis intervention techniques and provide both brief and long-term individual, group, and family therapies as appropriate to meet client needs.
• Act as a resource to clinical staff, offering guidance in psychosocial practices and providing education on mental health topics as necessary.
• Participate in clinical evaluations of oncology patients, assessing for depression or anxiety and providing necessary interventions.
• Engage in an on-call rotation for after-hours support and crisis intervention, ensuring availability for urgent patient needs when they arise.
• Present at educational conferences, sharing your expertise in psychosocial oncology and contributing to the advancement of practice standards.
• Contribute to community services through participation in educational programs, research, and volunteer initiatives, enhancing resources for cancer survivors and their families.

Patient Population / Client Focus

This position primarily focuses on adult and pediatric patients within the community health clinic environment who are dealing with chronic illnesses, mental health challenges, and complex social situations. Our aim is to empower these individuals and families by offering them the tools and support they need to navigate their health journeys effectively.

Qualifications

• A valid LCSW license is required to practice in the state of Oregon.
• A Bachelor’s Degree in Social Work or a related field is essential; a Master’s Degree is preferred.
• Proven experience in a clinical setting, especially with oncology and psychosocial interventions, is highly advantageous.
• Strong interpersonal skills and the ability to engage with diverse populations effectively.
• Proficiency in using electronic health record (EHR) systems to document assessments and care plans accurately.

Preferred Experience

• Experience in a community health clinic or hospital environment.
• Familiarity with oncology care and understanding of the psychosocial impacts of chronic illness.
• Previous involvement in educational or community outreach activities related to mental health and wellness.

Compensation and Benefits

The compensation for this position ranges from USD 70,000 to 100,000 annually, commensurate with experience and qualifications.
