Key Responsibilities\n

• Conduct thorough assessments of clients' counseling needs and develop individualized treatment plans aimed at addressing their substance use disorders.
\n
• Utilize the DSM (Diagnostic and Statistical Manual) criteria to assess clients for substance abuse diagnoses, ensuring appropriate levels of care based on ASAM (American Society of Addiction Medicine) criteria.
\n
• Prepare and implement substance abuse counseling segments within treatment plans, focusing on comprehensive care that reflects each client’s unique circumstances.
\n
• Develop discharge and continuum of care plans to facilitate smooth transitions to the next appropriate level of care, emphasizing long-term recovery.
\n
• Document all counseling and educational services, assessments, progress notes, and follow-ups in clients' clinical records, ensuring accuracy and timeliness.
\n
• Maintain an active caseload while managing case management duties and complying with utilization review requirements.
\n
• Coordinate support services for clients and their families, recording services provided to ensure holistic care.
\n
• Establish contact and maintain coordination with various referral sources, including legal and educational institutions, to support clients' recovery journeys.
\n
• Facilitate crisis interventions and necessary therapeutic measures in collaboration with the multidisciplinary team for assigned clients.

Patient Population / Client Focus\n

You will primarily work with patients facing co-occurring mental health and substance use issues in a hospice setting, recognizing the unique challenges those in this environment may encounter. Your expertise will be essential in helping clients navigate complex emotional landscapes and providing the support needed to promote recovery and dignity at this critical stage of life.

Qualifications\n

• A Bachelor’s Degree in Social Work or related field is required, along with a valid LCSW license.

• Demonstrated experience in substance use counseling, particularly with co-occurring disorders.

• Strong therapeutic boundaries and a commitment to professional ethics and cultural competency.

• Excellent communication skills, self-motivation, and ability to work independently in a remote environment.

Compensation and Benefits\n
