Regular business hours will typically frame the workflow, although occasional evening or weekend hours may be necessary to meet client needs and program demands.

Key Responsibilities\n

• Conduct thorough assessments to identify the unique social, emotional, and environmental challenges faced by clients, ensuring a comprehensive understanding of their situations.

• Develop and implement personalized service plans detailing specific goals, interventions, and methods for monitoring progress.

• Provide one-on-one counseling and therapeutic support to individuals and families coping with grief, loss, and other life transitions.

• Collaborate closely with clients to tailor their service plans and connect them with relevant community resources, enhancing their support networks.

• Offer immediate crisis intervention, risk assessments, and safety planning for clients in acute need.

• Build collaborative relationships with community organizations, ensuring a coordinated approach to service delivery.

• Engage in proactive community outreach efforts to strengthen public awareness about our services and foster partnerships that benefit our clients.

• Participate actively in team meetings, case conferences, and ongoing professional training to remain updated on best practices in mental health and social work.

Patient Population / Client Focus\n

In this role, you will work primarily with individuals and families facing significant life challenges, many of whom are dealing with terminal illnesses and the accompanying emotional and psychosocial stress. You will address clients’ needs holistically, focusing on enhancing their quality of life and supporting them during critical periods of their lives.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• An active and unrestricted state license as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
