Responsibilities

• Provide excellent patient care through collaboration with the healthcare team.
• Coordinate a continuum of care for a defined patient population from pre-admission through post discharge.
• Act as a liaison among patients, families, and the healthcare team to optimize outcomes.
• Serve as a consultant on specific patient items to the multidisciplinary team.
• Ensure that patients and families navigate efficiently through hospitalization.
