Work Environment\n

As a remote worker based out of Springfield, Massachusetts, you will have the flexibility to manage your schedule while maintaining a structured workweek, operating Monday through Friday from 9:00 AM to 5:00 PM. Key Responsibilities\n

In your role as a Clinical Social Worker, you will undertake a variety of responsibilities focused on ensuring holistic support for our clients. Your day-to-day tasks will include:

• Acting as a liaison between patients, families, and outpatient care providers to coordinate comprehensive care plans.

• Engaging with insurance companies to facilitate pre-certification and manage utilization reviews to ensure access to necessary services.

• Participating in the discharge planning and aftercare processes, ensuring clients transition smoothly from hospice care.

• Providing therapeutic meal support and modeling healthy relationships with food, assisting clients in achieving a balanced, nutritious lifestyle.

• Delivering empathetic and affirming support by understanding and honoring each client’s unique identities and backgrounds, ultimately enhancing their wellbeing.

Patient Population / Client Focus\n

This role primarily focuses on providing support to older adults facing end-of-life challenges, alongside their families and caregivers. Your understanding of geriatric care and the emotional complexities associated with hospice settings will be invaluable in helping clients navigate their circumstances with dignity and grace.

Qualifications\n

To be successful in this role, candidates must meet the following qualifications:

• A Master’s degree in Social Work from an accredited institution.

• A valid LICSW license in the state of Massachusetts.

• Demonstrated experience working with older adults in clinical or therapeutic settings.

Compensation and Benefits\n
