Key Responsibilities\n

• Conduct comprehensive and ongoing assessments of clients to determine their mental health needs, developing individualized treatment plans that support their journey towards wellness.

• Engage and communicate regularly with supervisors, direct care staff, and family members to share updates on client progress and necessary adjustments to treatment plans.

• Create and provide a tentative schedule for appointments, home visits, and progress observations, ensuring all activities align with client needs and program guidelines.

• Coordinate and conduct in-home visits focused on observations, chart reviews, and data collection from clients in their natural environments, promoting a client-centered approach to care.

• Monitor the effectiveness of treatment programs, ensuring all plans are correctly implemented and adjusted as necessary to meet evolving client needs.

• Participate actively in Interdisciplinary Team Meetings, contributing valuable insights and expertise relevant to client care strategies and program developments.

• Maintain meticulous records and documentation, including monthly and quarterly progress notes, ensuring compliance with legal and ethical standards.

• Act as a liaison between clients and the wider mental health community, providing advocacy and resources that support client personal and professional development.

• Assist in developing and conducting training sessions to certify staff in program implementation and documentation processes, ensuring the highest standards of care.

• Provide ongoing support through follow-up interviews and additional home visits as needed, remaining responsive to the changing requirements of each client.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field from an accredited institution.
