Position Overview

Breakthrough Rehabilitation & Treatment Center seeks a dedicated and passionate Physical Therapist to join our team in Red Bank, New Jersey. Key Responsibilities

• Ensure compliance with state practice acts and uphold the legal standards regulating physical therapy practice.
• Conduct thorough evaluations and assessments of patients to develop personalized treatment plans tailored to their specific functional limitations and rehabilitation goals.
• Administer treatment techniques including manual therapy, therapeutic exercises, and modalities based on the patient's individual needs and diagnosis.
• Monitor patient progress, identify changes in condition, and adjust treatment plans accordingly to optimize recovery and outcomes.
• Maintain clear and accurate documentation of patient evaluations, treatment sessions, discharge planning, and progress notes to provide a comprehensive overview of patient care.
• Collaborate and communicate effectively with interdisciplinary healthcare teams to ensure cohesive patient care and early recognition of potential complications.
• Engage in patient education, explaining treatment techniques and self-management strategies to promote independence and enhance recovery.
• Participate in facility meetings and contribute to continuous improvement initiatives aimed at eliminating barriers to access and enhancing service delivery.
• Complete required training to remain compliant with corporate standards, state regulations, and evolving healthcare practices.

Patient Population / Client Focus

Our facility specializes in providing physical therapy services to individuals with diverse orthopedic conditions, post-surgical rehabilitation needs, and chronic pain management issues. The therapist will work with a varied caseload, ensuring that interventions are evidence-based and align with the latest research to provide optimal patient outcomes.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• Current state licensure as a Physical Therapist (PT) in New Jersey.
• Strong clinical skills with a focus on outpatient orthopedic therapy.
• Current CPR certification.

Preferred Experience

• A minimum of two years of outpatient orthopedic experience preferred, demonstrating an ability to handle a diverse patient population.
• Experience in working with electronic health record (EHR) systems for documentation and patient management.
• Proven track record of patient rapport building and education.

Compensation and Benefits

We offer a competitive annual salary ranging from $100,000 to $120,000, commensurate with experience and qualifications. Our comprehensive benefits package includes:
