In this position, you will work remotely with the option to engage in outpatient office visits as needed. Key Responsibilities\n

• Conduct thorough initial assessments and ongoing evaluations to identify the specific needs of each patient.

• Maintain open communication with supervisors, healthcare teams, and family members to share progress updates and ensure cohesive care.

• Develop and propose a structured schedule for appointments, home visits, and observational reviews to monitor therapy effectiveness and client engagement.
\n
• Coordinate home visits to facilitate in-home observations, conducting chart reviews, and interviewing relevant staff to gather comprehensive insights for treatment planning.
\n
• Implement the designated program plans effectively, ensuring adherence to clinical best practices and ethical guidelines.
\n
• Engage in Interdisciplinary Team Meetings and other relevant forums to contribute to a collaborative treatment approach.
\n
• Document and complete monthly and quarterly progress notes with detailed records of each patient's treatment journey and outcomes.
\n
• Organize and facilitate training sessions to ensure that staff and team members are well-equipped to implement program plans and maintain proper documentation.
\n
• Conduct follow-up assessments, including interviews and observations, to continue supporting each client’s unique journey towards wellness.
\n
• Act as a liaison between clients, families, and the mental health community, advocating for support and resources in alignment with development goals.

• Assist in developing training materials and procedures aimed at enhancing program quality and efficiency for clients.

Qualifications\n

• Possession of a Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• Active and unrestricted licensure as a Clinical Professional Counselor (LCPC) in the state of Illinois is mandatory.
