• What You'll Do As a Home Health Registered Nurse, you'll provide skilled nursing care that allows patients to recover safely and comfortably in their own homes. You'll partner with physicians, therapists, caregivers, and interdisciplinary team members to develop personalized care plans that improve outcomes and enhance quality of life.

Develop, implement, and adjust individualized plans of care.

Educate patients and caregivers on disease management, medications, treatments, and self-care.

Collaborate with physicians and the interdisciplinary care team to coordinate services.

Accurately document clinical findings and care provided.

Current, unrestricted Registered Nurse (RN) license

Minimum of one (1) year of nursing experience in a clinical setting

Compassionate, patient-centered approach to care

Reliable transportation, valid driver's license, and current auto insurance

Willingness to travel throughout the assigned service area

Physical Requirements : Sit, stand, bend, lift, and move intermittently throughout the workday. Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.

Associate or Bachelor's Degree in Nursing

Previous Home Health or Hospice experience

Experience with Homecare Homebase (HCHB)
