Physiotherapist I - Home Care Rehab

We are seeking a dedicated and enthusiastic Clinical Physical Therapist to join our growing team at Phoenix Physical Therapy, providing high-quality home care to patients in the beautiful San Diego, California area. This full-time position focuses on delivering specialized physical therapy services, ensuring patients achieve optimal functional independence and improve their quality of life. This role will be performed onsite in a home care setting, providing flexibility and the opportunity to directly engage with patients in their own environments. As a Clinical Physical Therapist, you will navigate various home settings, utilizing clinical assessment skills and creative treatment approaches tailored to each patient's unique needs. Your work hours will be structured Monday through Friday, from 9 AM to 5 PM, allowing for a balanced professional and personal lifestyle.
Administering direct patient care as per the written and signed prescriptions from referring physicians or state-authorized practitioners.
Developing and implementing personalized treatment programs designed to achieve specific rehabilitation goals.
Monitoring and observing patient status and responses throughout treatment, making adjustments to therapeutic interventions as clinically necessary.
Ensuring compliance with departmental policies by accurately documenting all patient care activities.
Participating in quality improvement initiatives, including chart reviews and professional development activities.
In this role, you will primarily work with patients recovering from diverse conditions including post-surgical rehabilitation, sports injuries, neurological disorders, and chronic pain management. Current California state licensure as a Physical Therapist in good standing.
A Bachelor's Degree in Physical Therapy or a related field.
A minimum of 2 years of relevant experience in a clinical setting, ideally with a focus on home care.
Experience in a home care setting.
