Caseworker/Mental Health Clinics

To provide support and information to patients and their families to maximize the psychosocial functioning and adjustment of the patient in End Stage Renal Disease (ESRD) including but not limited to financial programs, medication assistance programs, vocational rehabilitation programs, transportation resources, mental health and chemical dependency resources, and home health/homemaking agencies.

Master's Degree in Social Work (Required)

With a licensed Social Worker will conduct initial patient psychosocial assessment within 30 days of admission, within a re-assessment to be conducted 90 days after the initial assessment and prepare the appropriate documentation in electronic medical record (EMR).

With a licensed Social Worker will develop and implement a goal-directed plan of care in collaboration with the interdisciplinary team based on changing needs.

With a licensed Social Worker will develop treatment and psychosocial goals for initial plan of care and update the plan of care as outlined by the clinic/center's Governing Body for stable and unstable patients.

With a licensed Social Worker will complete and document psychosocial evaluations quarterly and annually; document and monitor ongoing psychosocial issues and patient goals.

With a licensed Social Worker will complete a quality of life measure (KDQOL-36) as determined by Carle Health for each patient.

With a licensed Social Worker will analyze and provide information and facilitate referrals to community, private, state and federal agencies based on psychosocial needs (e.g. financial programs, medication assistance programs, Vocational rehabilitation programs, transportation resources, mental health and chemical dependency resources and home health/homemaking agencies).

With a licensed Social Worker will provide short-term crisis intervention and individual and family counseling and support as appropriate.

With a licensed Social Worker will provide education to clients and caregivers on the hospice philosophy, informed consent, Medicare/Medicaid benefits, client rights and responsibilities, Advance Directives, home safety, financial issues and communicating concerns

provide support, information and assistance in treatment planning; provide assistance to patient and family in adjusting to dialysis and altered lifestyle, provide resource information.

Educates and assists with nursing home placement, in home care planning and respite/acute admissions.

Educates and assists with Advance Directive planning including, Living Will, Power of Attorney, Surrogate and/or guardianship process, DNR and other end of life issues as appropriate.

Reports any questionable medical or other professional practice, as well as any known abuse situations to the Clinical Manager.

Remains current with regulatory, legislative and reimbursement mandates.

Remains current with available dialysis resources.

Assumes responsibility for own personal and professional development and maintenance in the area of ESRD & dialysis modalities.

Participates in the orientation program for new staff, volunteers and students as assigned.

Ensures quality and safe delivery of care.

Observes, evaluates and brings to team conferences information regarding psychosocial, physical and financial conditions affecting the client and caregiver.

Participates in developing and modifying a client/caregiver plan of care in collaboration with other team members.

Develops and maintains contact with appropriate community agencies and services in order to promote interagency cooperation.

Initiates and participates in family conferences when problems are identified to ensure quality care.

Practices and reflects knowledge of HIPAA, TJC, DNV, OSHA and other federal/state regulatory agencies guiding healthcare.

Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff.
