Job Overview Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health to support patients in Evanston, Skokie and surrounding areas in Lincolnwood, Illinois.

Hours: 40 hours per week (8‑hour shift)
Employment Type: Staff (Full‑time)
Responsibilities Home health registered nurse case managers lead and collaborate to provide management and delivery of patient care plans. You will support patients by delivering individualized home health visits, ensuring compliance with care plans, and coordinating with interdisciplinary teams.
Qualifications RN license in the state where you will work
Graduate of an approved school of practical nursing
One year of RN experience with the clinical competence to deliver quality patient care
Experience using tablets, mobile phones, and electronic medical record (EMR) software
Additional Requirements Successful completion of a drug screen prior to employment (includes medical and recreational marijuana)
Legal authorization to work in the United States
