Physical Therapist - Home Health Care (PT)

Pride Health is actively hiring a Physical Therapist (PT) to join our client's Home Health Team.

Job Summary We are seeking an experienced and compassionate Physical Therapist (PT) to provide high-quality home health services to patients in their homes.

The Physical Therapist will develop and implement individualized plans of care to help patients improve mobility, strength, balance, safety, and overall functional independence.

Details Weekly Gross Pay:2,570.00

Schedule:Monday-Friday, 8:00 AM-4:30 PM

On-Call/Weekend Requirement:None

Mileage reimbursement:Patient Care Setting:Home Health

Key Responsibilities Provide skilled home health PT services, including gait/balance training, strengthening, mobility improvement, and home safety interventions based on individualized plans of care.

Maintain accurate clinical documentation, coordinate with the care team, monitor patient progress, and meet the 30-point weekly productivity expectation.

Required Qualifications Current and active Physical Therapist (PT) license in the State of Ohio.

Minimum 1 year of Home Health Physical Therapy experience.

Strong clinical assessment, treatment planning, and patient education skills.

Preferred Qualifications Experience with Homecare Homebase (HCHB) EMR.
