This position is based in Charlottesville, a vibrant city known for its beautiful landscapes and community-oriented lifestyle. The home care facility allows therapists the flexibility to provide treatment in a familiar environment that is often most comfortable for patients. Our team works collaboratively to share insights and best practices, fostering a culture of continuous improvement and optimal patient care. With a structured schedule from 7:30 am to 3:30 pm or 8:00 am to 4:00 pm, Monday to Friday, you can maintain a healthy work-life balance.

Develop and implement tailored treatment plans that employ a variety of therapeutic techniques, including physical exercises, manual therapies, and the use of assistive devices.

Continuously track and document patient progress, making adjustments to treatment plans based on ongoing evaluation and patient feedback.

Participate in interdisciplinary collaboration with other healthcare professionals to ensure holistic patient care.

Maintain your professional knowledge by attending workshops, engaging with professional literature, and participating in relevant training opportunities.

This role focuses on diverse patient demographics, from children to elderly individuals requiring rehabilitation following various medical conditions, including orthopedic surgeries, neuromuscular disorders, and post-injury recovery. As a physical therapist, your impact is profound as you guide patients through tailored recovery strategies in the comfort of their homes, fostering both physical mobility and emotional resilience.

A Doctorate in Physical Therapy (DPT) is required to ensure a strong foundation in therapeutic practices.

Possession of a valid state license in Physical Therapy, affirming compliance with state educational and professional standards.

A minimum of two years of experience in physical therapy, demonstrating a solid understanding of best practices and treatment methodologies.

Proficiency in word processing and computer skills to maintain patient documentation and communicate within our scheduling systems.

Experience working in home healthcare settings, showcasing an understanding of the unique dynamics and challenges involved.
