Key Responsibilities\n

• Provide direct clinical services to clients experiencing major mental illness and severe emotional disorders, both in-person at the hospice center and virtually.

• Establish positive rapport and trust with clients, enhancing their engagement and adherence to treatment plans.

• Conduct comprehensive assessments and screenings to gauge clients' mental health needs, and develop individualized care plans.

• Collaborate with interdisciplinary teams, including healthcare providers, to implement integrated care plans that address clients' holistic needs.

• Offer a range of brief, focused prevention, treatment, and recovery services tailored to each client’s unique circumstances.

• Participate actively in team meetings and case reviews to ensure alignment and collective focus on client outcomes.

• Maintain adherence to ethical guidelines and organizational standards to uphold trust in patient care.
\n
• Identify and report unsafe situations within the care environment, always prioritizing safety and well-being.

• Facilitate communication among clients, family members, and providers to support continuity of care.

This role focuses on a diverse clientele, particularly individuals who are in hospice care, experiencing complex emotional and psychological challenges. Many clients have multi-faceted needs arising from terminal illnesses, making the provision of empathetic and informed mental health care essential. Qualifications\n

• Must possess a valid LCSW license for the state of New Jersey.

• A Bachelor’s Degree in Social Work or a closely related field is required; a Master’s degree is preferred.

Compensation and Benefits\n

The salary range for this position is competitive and lies between USD 70,000 and 85,000 annually, commensurate with experience.
