This role offers a flexible remote work setting, allowing our counselors to engage with patients from the comfort of their homes. While collaborating closely with a dedicated primary care team, counselors can manage their schedules around patient appointments. Key Responsibilities\n

• Execute intake assessments for patients focusing on diagnostic and functional evaluations, providing comprehensive recommendations for treatment plans to primary care providers.

• Conduct brief, focused therapeutic interventions aimed at addressing immediate mental health needs.

• Evaluate the appropriateness of patient referrals between primary care settings and specialty mental health facilities to ensure optimal care pathways.
\n
• Maintain continuous follow-up with patients, assessing their progress and making necessary modifications to their treatment approach.
\n
• Develop relapse prevention strategies for at-risk patients and assist them in maintaining stable functioning in their daily lives.
\n
• Monitor the delivery of health services related to behavioral health care, ensuring seamless coordination among treatment providers, both within and outside the clinic.

• Document patient progress and diagnostic information meticulously to maintain accurate treatment records.
\n
• Actively collaborate with primary care providers to communicate patient needs, achievements, and assist in formulating cohesive treatment plans.

• Empower patients by equipping them with self-management skills and educational resources, fostering their role in the treatment and recovery process.
\n
• Provide insights and consultation assistance to clinic management and team members regarding behavioral services and program evaluation.

• Assist patients in adhering to medical treatments initiated by primary care providers, including strategies for managing potential medication side effects.

Patient Population / Client Focus\n

This position primarily involves working with a diverse patient population experiencing various mental health challenges, including anxiety, depression, trauma-related issues, and chronic stress. Counselors will play a crucial role in addressing not only individual mental health needs but also the broader psychosocial factors that impact their overall well-being. Qualifications\n

• A Master’s Degree in Clinical Counseling or a related field is required.

• A valid and current LPC (Licensed Professional Counselor) license.

• Comprehensive knowledge of mental health assessment, treatment planning, and psychotherapeutic methods, including but not limited to DSM-IV diagnoses and developmental psychopathology.

• Strong capacity to evaluate individual patient needs and the ability to establish effective therapeutic relationships with patients and their families.

• Possess mental and physical health adequate to fulfill job responsibilities.

• Thorough understanding of community resources and support systems available to patients.

Preferred Experience\n

• Experience in a community health setting or integrated health model is highly desired.

• Bi-lingual proficiency in English and Spanish is advantageous, enhancing our service delivery to a broader demographic.

Compensation and Benefits\n

We offer a competitive salary range of USD 87,000 – 105,000 per year, commensurate with experience and qualifications.
