Key Responsibilities\n

• Conduct thorough assessments to identify patients’ emotional, psychological, and social needs, establishing a comprehensive understanding of their background and care goals.

• Deliver individual and family therapy sessions remotely, employing evidence-based modalities such as cognitive-behavioral therapy, solution-focused therapy, and related approaches tailored to the unique circumstances of terminally ill patients.

• Develop and oversee personalized treatment plans in collaboration with patients, continuously adjusting them based on ongoing feedback and progress monitoring.

• Engage in interdisciplinary collaboration with healthcare professionals — including doctors, nurses, and hospice staff — to provide holistic care that addresses patients’ mental, emotional, and physical needs.

• Ensure strict adherence to ethical guidelines and patient confidentiality while maintaining detailed documentation of all interactions and treatment plans.

• Actively participate in ongoing professional development by attending workshops, training sessions, and relevant seminars to stay updated on the latest best practices in hospice care.

• Foster a supportive therapeutic alliance to encourage trust, empowerment, and healing as patients navigate their end-of-life journeys.

Patient Population / Client Focus\n

This role primarily focuses on individuals who are facing terminal illnesses and their families. Your work will involve providing emotional support, facilitating difficult conversations about end-of-life issues, and developing coping strategies that assist patients and their families in managing grief, loss, and transition.

Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

• Active state licensure as a Licensed Clinical Social Worker (LCSW).

• At least two years of direct clinical experience, preferably in a hospice, palliative care, or similar environment.

• Proven clinical skills in assessment, intervention planning, and implementation, with a strong understanding of end-of-life issues.

Compensation and Benefits\n
