Work Environment

This role is onsite and involves home care, allowing you to work directly with patients in their own environments. You will collaborate closely with physicians and other healthcare professionals to implement individualized treatment plans. Key Responsibilities

• Deliver comprehensive, patient-centered care by managing the therapeutic process for an assigned group of patients, in collaboration with the patient care team.
• Conduct detailed patient assessments and develop personalized care plans aimed at achieving treatment goals through active patient engagement.
• Administrate targeted physical therapy interventions at patients' homes, employing best practices to align with the physician's prescribed treatment plan.
• Assess and evaluate patient progress in therapeutic and functional statuses to adapt care plans as necessary.
• Educate patients and their families or caregivers on therapeutic exercises, equipment use, and ongoing care strategies to be utilized outside of therapy sessions.
• Organize, prioritize, and manage caseloads effectively while determining daily staffing needs and facilitating seamless patient hand-offs.

Patient Population / Client Focus

Our patients include a wide range of individuals requiring physical therapy for various orthopedic conditions. This position allows you to work with both adults and children, providing a unique opportunity to impact multiple age groups. Qualifications

• Required: Valid Physical Therapy License in California.
• Required: Doctor of Physical Therapy (DPT) degree.
• Must complete Basic Cardiac Life Support (BCLS) certification within 30 days of hire.
• No prior experience is necessary; new graduates are encouraged to apply.

Preferred Experience

While no experience is required, candidates with prior internships or clinical rotations in home health settings or orthopedics will be viewed favorably. Any exposure to interdisciplinary teamwork and patient care within the healthcare system will also enhance your application.

Compensation and Benefits

The salary for this position ranges from USD 100,000 to 120,000 annually, accompanied by a comprehensive benefits package. Our offerings include:
