Key Responsibilities\n

As a Licensed Clinical Social Worker for Horizon Mental Care Services, your responsibilities will include:

• Conducting thorough clinical evaluations to determine patients' psychosocial needs and developing individualized care plans that meet these requirements.

• Facilitating group therapy sessions and psychoeducational workshops, guiding patients through grief, coping strategies, and the emotional stresses associated with hospice care.

• Completing comprehensive discharge sessions to ensure that patients and their families understand coping mechanisms, transition plans, and community resources that will support them post-discharge.

• Confirming discharge appointments with patients and caregivers beforehand to ensure a smooth transition.
\n
• Engaging in continuous communication with referral sources and outpatient providers to maintain continuity of care at various stages of the patient's journey, including admission and discharge.

• Managing patient transfers to higher levels of care when necessary, ensuring seamless transitions that prioritize patient safety and comfort.

• Participating in clinical outreach activities and program tours to enhance community awareness and access to our hospice services.

Patient Population / Client Focus\n

Your primary focus will be providing compassionate care to terminally ill patients within the hospice framework. This role requires sensitivity to patients' emotional and psychological needs, emphasizing a person-centered approach that respects their dignity and wishes. You will also work closely with family members, providing them with support and resources to navigate the complexities of grief and loss.

Qualifications\n

To be considered for the LCSW position, you must meet the following qualifications:

• A Master’s degree in Social Work from an accredited institution.

• Active and unrestricted licensure as a Clinical Social Worker (LCSW) in the state of California.
