Key Responsibilities\n

• Independently manage an assigned caseload of patients, performing thorough evaluations to assess their social, emotional, financial, and cultural factors that influence their health and treatment plans.

• Utilize a systematic approach to provide a full range of therapeutic and intervention strategies aimed at resolving psychosocial barriers to care, including those related to grief, mental health issues, and family dynamics.

• Champion the psychosocial needs of high-risk patients by developing and actively overseeing treatment plans, addressing protective services, advance directives, and life care decisions.

• Facilitate crisis interventions for patients and families, providing immediate support to help them navigate critical encounters with healthcare services.

• Coordinate with case managers and other healthcare professionals to optimize resource allocation, ensuring all psychosocial and socioeconomic needs are met prior to discharge.

• Educate the healthcare team about the social worker's role, enhancing understanding and collaboration around psychosocial patient care.

• Supervise and mentor Social Work Interns, MSWs, and volunteers, fostering professional development and enhancing quality of care through effective practice standards.

• Implement timely psychosocial services to prevent avoidable delays in treatment plans and discharge processes.

• Participate in team rounds and patient care conferences, contributing to care coordination activities, especially for children requiring discharge planning.

Qualifications\n

Compensation and Benefits\n

This role offers a competitive salary range of USD 120,000 to 130,000 per year, commensurate with experience and qualifications.
