Key Responsibilities

• Direct comprehensive physical therapy treatments aimed at preventing disabilities, relieving pain, and restoring muscle function, closely following individualized care plans.
• Oversee and mentor physical therapy assistants to ensure adherence to therapeutic plans.
• Conduct thorough evaluations of each patient's physical capabilities using diagnostic tests to assess muscle, nerve, and joint functionality.
• Analyze patients' home environments and recommend necessary modifications or equipment enhancements that promote independence and safety.
• Deliver therapeutic exercises, instruct patients in both manual and non-manual techniques, and engage them in rehabilitation activities designed for their specific conditions.
• Administer treatments involving physical agents such as traction and massage while keeping up with best practices and current techniques.
• Evaluate, fit, and make recommendations concerning prosthetic and orthotic devices in collaboration with the prescribing physician.
• Participate actively in multidisciplinary team meetings, contributing to holistic care strategies for each patient.
• Maintain accurate clinical documentation, including evaluations, care plans, progress notes, and discharge summaries, ensuring compliance with regulatory requirements.
• Consult with patients and their families regarding treatment plans and advise on device usage for continuity of care at home.
• Stay accessible to the physical therapy assistant during treatment sessions, ensuring all therapy services meet required standards.

Patient Population / Client Focus

You will work with a diverse patient population, including individuals recovering from surgeries, those with chronic pain, and patients requiring assistance for mobility challenges. Your role is vital in helping clients regain their independence and improve their overall quality of life through tailored therapy programs in the comfort of their homes.

Qualifications

• Must be a graduate of an accredited physical therapy program.
• Current physical therapy licensure in good standing in Virginia or applicable state.
• Minimum of one year of experience in physical therapy settings, preferably in acute care or home care.
• Certification in CPR and a valid driver's license with automobile insurance.
• A strong passion for clinical excellence, quality care, and integrity is essential.

• Experience using electronic medical record (EMR) systems for efficient documentation and care management.
